How this calculator works

Manufacturer prices come from the makers' own cash programs: NovoCare, LillyDirect and TrumpRx. Coverage figures come from CMS and the savings card terms. Program prices come from our register of online programs, checked against each program's own checkout. Each figure names its source and the date we checked it.

The math is simple. The monthly cost is the published price for a 28–30 day fill. The 12-month cost is that price times 12, with no discount.

Limits: savings card terms change each year and cap the yearly benefit. Starting prices, such as $199 for the first two Wegovy fills or $299–$449 for Zepbound by dose, depend on the program's rules. Medicaid coverage changes by state. Program prices change often, so check the price at checkout.

Cost questions

Almost nobody pays list. Manufacturer cash programs sell the same pens, vials and tablets for $149–$499 a month, savings cards cap commercial copays near $25, and online programs sell compounded semaglutide for less than $100 a month. List price mostly matters when you leave a program’s rules.

No. Federal anti-kickback law bars manufacturer copay cards on Medicare, Medicaid and Tricare. Medicare patients qualify instead for the $50 a month GLP-1 copay program phased in during 2026, or can pay cash through NovoCare, LillyDirect or TrumpRx. Pure cash programs are allowed.

We show the cheapest program we recommend for your answers. A program must score at least 7.5 out of 10 in our review, sell your medication on a live plan at a price we verified, and have no open critical safety flag. A company that runs two storefronts counts once. We rank on what you really pay each month: where a plan leaves the medication out, we add the maker’s price for it. Some links pay us a fee. The fee does not change the order.

Program prices include the visit, and most include the medication too. Each card says when the medication is extra. Manufacturer programs (NovoCare, LillyDirect, TrumpRx) price the medication only. You still need a prescription, from your own doctor or from an online program.

Semaglutide’s negotiated Medicare price, announced near $274 a month, starts on January 1, 2027. With the 2026 $50 copay program, what you pay should fall further. Final CMS guidance is still pending.

Usually, but the gap is smaller. Compounded programs run about $59–$199 a month. Brand-name cash prices are $149–$299 (Wegovy tablets), $299–$449 (Zepbound) and $349 (Wegovy pen). Compounded supply depends on pharmacies that work within FDA rules, so check the program’s safety record first.
